    An appropriate team member reviews the home situation and the kind of support that may fit.

    Agree on a care plan

    Before care begins, the proposed services, schedule, responsibilities, and communication approach are explained.

    Begin and review care

    Care starts after the required arrangements are in place, with check-ins as needs and routines evolve.

Before an assessment

A little preparation makes the conversation easier.

Bring only what is relevant, and use an approved secure method for sensitive information.

  • A short list of the routines or tasks where help may be needed
  • The individual’s goals, preferences, and concerns
  • A current medication list or discharge instructions, if requested securely
  • Names of family members or professionals involved in decisions
  • Questions about schedule, cost, coverage, and communication
